Consultation Only Appointments are for patients that do not need or want testing or therapies.
This is an appointment to talk with the doctor and ask questions.

Our email is used only for:
Sending in lab results.
Sending in medical records.
Giving the doctor an update on your status per his request of you.
If you have a simple Q&A.
If you have multiple questions for the doctor, please call the office or schedule an appointment and bring your questions with you to your appointment.

Regular Appointment - $145 (30 minutes)
Extended Appointment - $290 (60 minutes)
Advanced Appointments - $450 (90 minutes)

ALL LAB RESULTS AND MEDICAL RECORDS MUST BE SENT IN PRIOR TO YOUR APPOINTMENT THROUGH EMAIL, MAIL, OR FAX TO ALLOW SUFFICIENT TIME FOR THE DOCTOR TO THOROUGHLY REVIEW THEM BEFORE YOUR SCHEDULED APPOINTMENT.

Flat-Fee Phone Appointment - $145 (30 Minutes)
If a longer appointment is needed, please schedule another appointment with our front desk.
If you have medical records or lab results for the doctor to review, please send them in prior to your phone appointment.
Consultation Only Appointments do not include any testing, labs, or therapies.
All payments are due at the time of service.
Mountain Family Wellness is self-pay. We do not bill insurance.
There is a $45 no show fee. Please cancel 24hrs before appointment to avoid the fee.
